Please complete the following application for membership. Each applicant must complete this form with the exception of children under 18. Parents/guardians must register each child that will use the facilities. A parent/guardian signature is required for persons 17 years and younger. Children under the age of 14 must be accompanied by an adult at all times, unless participating in a program offered by FLCC. All membership fees are non-refundable.

FLCC and its members, staff, volunteers and visitors are required to uphold the following code of conduct. Any violations or failure to comply with the rules, regulations and the contents of this agreement may result in suspension or expulsion from the use and/or privileges to utilize FLCC. The following are not permitted inside or outside the facilities of FLCC:
- Illegal drugs, alcohol or weapons
- Smoking
- Abusive or insulting language
- Fighting, theft, or vandalism
All members are responsible for their own personal items.
By submitting this application, I acknowledge and accept to comply with FLCC’s code of conduct. I acknowledge the risk inherent to participation in recreational activities. I agree to hold harmless GMCHC and FLCC, its officers, employees, and volunteers from any and all claims from bodily injury and/or property damage which results from my wrongful judgment, failure to abide and comply with FLCC policy and participation in any and all activities sponsored by the FLCC.
